I/we own the above property. Please process the electric account in the following manner:

If the electric account at the above listed service location is in the name of my tenant and the tenant requests the discontinuance of electricity, I authorize Hull Municipal Lighting Plant to automatically return the account to my name, thereby avoiding interruption of electric service.

I understand that the standard turn-on charge of $40 will be reduced to $20 for this service, which I agree to pay each time the account is transferred to me name.
